A Successful Case of Renal Transplantation from Deceased Donor Performed 7 Years after Radioactive Seed Implantation Therapy for Localized Prostate Cancer.

There has been a significant increase in the number of patients with Prostate Adenocarcinoma (PCa) over the last decade, leading to a substantial rise of pre-transplant patients with a history of cured PCa.

Currently, Radical Prostatectomy (RP) and brachytherapy using isotope seeds are good alternatives for treatment of localized low risk PCa. In this case, we performed a cadaveric renal transplantation for a patient who 7 years previously underwent radioactive seed implantation therapy for low risk PCa, as defined by the D’Amico classification system.

The patient was a 66-year-old man with a history of hemodialysis for 24 years. Although radiation therapy can cause unfavorable inflammatory changes such as adhesions, we did not encounter any intraoperative difficulty, and the postoperative course has been uneventful and serum prostate specific antigen has remained undetectable even about 2 years after the transplant

Prevalence of Surgical Site Infections in Non-Diabetic Patients Undergoing Major Surgery at St. Francis Hospital Nsambya

Surgical site infection (SSI) is an infection occurring in an incision wound within 30 days of surgery and significantly effects patient recovery and hospital resources.

To determine the prevalence of SSI among non-diabetic patients undergoing major surgery at St.Francis hospital Nsambya. This was a Cross-section prospective study, the study was carried out at St. Francis Hospital Nsambya in Kampala. This is one of the tertiary level referral and teaching hospital for MKPG Nkozi University. It has a bed capacity 342 beds.

Approval to conduct the study was obtained from the institutional review board prior to commencement of data collection. Of the 140 patients enrolled in the study, male were 90 (64.3%) giving male female ratio 1.8:1. Surgical site infection (SSI) was seen in 40 (28.6%) patients and these were.

Proximal interphalangeal (PIP) joint injuries are commonly seen in athletes and especially the pilon fractures are quite common due to axial loading injuries.

It is associated with volar lip fragment with dorsal subluxation of middle phalanx with varying degree of comminution of the base of middle phalanx.

Varied treatment options have been described ranging from extension block splint, external frame finger distraction and open reduction internal fixation. The key is in restoring the joint congruity with early controlled mobilisation thereby preventing joint stiffness or deformity.

Fortification of hepatic progenitors from the emergent human liver, with minimum rejection in transplantation remains a major dare for the improvement of therapeutic approaches to liver disorders.

Human fetal liver derived hepatic progenitor’s cells recommend an imperative therapy for clinical application. Several investigators have shown that decreased phenotypic expression HLA-II markers in the EpCAM +ve cells isolated from the second trimester fetal liver.

Life-threatening Capillary Leak Syndrome in an Adult with Refractory Acute Myeloid Leukemia during Allogeneic Transplantation: a Case Report and Review of Literature

Although allogeneic hematopoietic stem cell transplantation (allo-HSCT) offers the possibility of cure for hematological malignancies, various complications have been described. Capillary leak syndrome (CLS) has been previously observed in HSCT patients.

CLS is a rare disease characterized by recurrent episodes of generalized edema and severe hypotension along with hypoproteinemia. Case Report: A 27-year-old Chinese man, diagnosed with refractory acute myeloid leukemia, was treated with a haploidentical stem cell transplant combined with an unrelated umbilical cord blood unit.

The patient developed fatal CLS during the 9th day of the conditioning therapy. Since it is difficult to distinguish between CLS and other early complications during allo-HSCT, our report highlights the need for rigorous investigation of identifying CLS and the increasing need of insightful diagnosis to manage any incidence of CLS.

Evaluation on the Effectiveness of the Different Modes of Publicity for Tissue Donation among Healthcare Professionals in Singapore

In Singapore, the donation of tissues is governed by the Medical (Therapy, Education and Research) Act. For past years, our tissue banks have engaged in various publicity methods. The objective of this study is to evaluate the effectiveness of our publicity strategies among healthcare professionals.

A 6-questions survey was conducted among doctors, nurses, allied health and support services staff from Singapore General Hospital and National Heart Centre Singapore. Sample size was calculated in accordance to the occupation ratio.

Of the 450 respondents who completed the survey, most did not see, read or heard about tissue donation. Newspaper articles were their main source of information amongst those who encountered tissue donation message. All categories of staff concurred that presentations by tissue banking experts is the best way to convey the message.

Use of Fascial Closure Needle Versus Staples for Mesh Fixation in Laparoscopic Transabdominal Preperitoneal Hernioplasty

Recently, it is being suggested in the literature that postoperative pain after TAPP is mostly attributed to the use of staples in mesh fixation.This study aims to compare the value and cost effectiveness of using facial closure needle versus the use of staples in the fixation of prosthetic meshes in laparoscopic tinguinal hernia repair (TAPP).

Sixty male patients suffering from inguinal hernia with a mean age 22.7 years ranging from 18 to 30 years were enrolled in this study mesh fixation using fascial closure needle was done in 30 cases and using staples in another other 30 cases.

Full physical assessment for the patients was performed and any postoperative complications such as early or late post-operative pain or burning sensation, hyperesthesia, wound infection, seroma, hematoma or recurrence were recorded.

Therapeutic Potential of Heme Oxygenase 1 in Ischemia Reperfusion Injury

Potential of Heme Oxygenase

Ischemia/reperfusion injury is a reaction occurring after successful treatment of an acute myocardial infarction or in the setting of solid organ transplantation. After successful reperfusion of a previously occluded vessel or a transplanted organ, an additional loss of otherwise vital cells may occur, the so-called lethal ischemia-reperfusion injury.

This lethal reperfusion injury is based on rapid tissue oxygenation, leading to a release of reactive oxygen species (ROS) and inducing oxidative stress, endothelial cell activation and inflammation.

Experimentally the reperfusion injury can be attenuated via ROS-scavenging, vessel stabilizing and anti-inflammatory interventions. Cytoprotective genes, such as heme oxygenase-1 (HO-1), offer a therapeutic approach to address this problem. Therefore, this review will focus on the beneficial effects of HO-1 in ischemia/reperfusion injury.

Prostate cancer (PCa) represents a common malignancy among elder males and one of the leading causes of cancer mortality. It may present with a variety of clinical behavior, including tumors of very low clinical significance but also highly aggressive tumors with increased risk of relapse after initial treatment.

Nowadays, many tumors traditionally treated either by radical prostatectomy or by external beam radiation therapy are considered of low clinical significance and such patients are placed under active surveillance protocols with purpose to reduce overtreatment.

As a result, in the “active surveillance” era, there is a need of establishing strong prognostic markers identifying aggressive tumors as well as clinical significant tumors even among these initially characterized of low or intermediate risk.

Our generation of orthopaedic surgeons starts to examine more and more sport related injuries in our daily practice. Acromioclavicular joint (ACJ) injuries are one of them. ACJ injuries rather frequent in athletes as well as the ordinary young, active people because of falls, sports injuries, and accidents.

Primary Non-Hodgkin’s Lymphoma of The Nasal Cavity: Unusual Site and Unusual Presentation

Non-Hodgkin’s lymphoma (NHL) is a heterogeneous subgroup of lymphomas with characteristic morphological and molecular features. Although lymph nodes are the most common organ of involvement, such as liver, stomach, intestine and bone marrow may also be affected on occasions. Primary non-Hodgkin’s lymphoma (NHL) of the nasal cavity is an extra nodal form of NHL characterized by association with Epstein-Barr virus (EBV) infection, predominance of T-cell phenotype and a worse prognosis compared to nodal lymphoma.

Although relatively common in China, nasal NHL is considered a rare entity in other countries of South East Asia. We are presenting a case of nasal NHL which was initially misdiagnosed as inverted papilloma and treated with surgical resection.
